Quick answer: two full items, no cheap component
League Classic 26.19 has exactly two purchasable Grievous Wounds items: Executioner's Calling for 1,900 gold and Morellonomicon for 2,200. Neither recipe grants anti-heal before the finished item, and neither item upgrades further. Choose a carrier who can complete the item without wasting its combat stats and can keep reaching the champion who will receive the healing.

Wound the champion receiving the heal
Both items reduce incoming healing and regeneration on the wounded champion by 50%; they do not reduce shields. The debuff belongs on the recipient, not automatically on the healer. If Soraka is about to heal her carry, wounding Soraka does not reduce the healing received by an unwounded carry. Identify whose health bar will rise, then keep the effect on that target through the recovery window.

Executioner's Calling needs repeatable attacks
Executioner's Calling combines Avarice Blade, Long Sword and 700 gold into 25 Attack Damage and 20% Critical Strike Chance. Basic Attacks against enemy champions apply 50% Grievous Wounds On-Hit for 1.5 seconds. The short duration rewards a carrier who can safely attack the correct target again; ordinary spell damage alone does not satisfy the Basic Attack trigger listed by the client.

Morellonomicon needs low-health magic damage
Morellonomicon combines Lucky Pick, Faerie Charm, Fiendish Codex and 435 gold into 75 Ability Power, 12 Mana Regen per 5 seconds and 20% Cooldown Reduction. Magic damage to an enemy champion below 40% Health applies 50% Grievous Wounds for 4 seconds. This is broader than spell damage, but the tooltip does not say whether a hit that crosses the 40% line counts, so do not plan around that undocumented edge case.

Damage type alone is not enough. Executioner's fits an AD/Crit buyer who can repeatedly Basic Attack the recipient, including before that target falls below 40% Health. Morellonomicon fits an AP user who can tag the recipient with magic damage after the threshold and use the four-second window. Before finishing either, ask whether healing will decide the next fight and which normal power spike the full purchase delays.

Miss Fortune's active Impure Shots makes her attacks apply 50% Grievous Wounds for 3 seconds during its six-second attack-speed window. Tristana's active Explosive Shot applies 50% during its five-second effect. Katarina's live Death Lotus record applies 50% for 3 seconds. Fizz, added in 26.19, puts the same 50% on his attacks for the 5 seconds of Seastone Trident's active; how long each wound lasts is not printed. The current Classic Ignite tooltip confirms healing reduction but exposes no live percentage or duration; do not import those numbers from old data.

Do not import the modern anti-heal shop
Classic Thornmail costs 2,200 gold, grants 100 Armor and reflects 30% of incoming pre-mitigation Basic Attack damage as magic damage; it does not apply Grievous Wounds. Bramble Vest is not in the Classic shop, and neither are the modern Mortal Reminder, Oblivion Orb, Chempunk Chainsword or Chemtech Putrifier routes. Tanks and low-budget supports often need a teammate, Ignite or a champion kit to carry the effect instead.

Match the answer to the recovery pattern
Mundo's Sadism is a long regeneration window, so repeated access matters. Against Soraka, Alistar or Shen, wound the ally receiving the heal rather than reflexively hitting the caster. Fiddlesticks Drain stops dealing damage and healing when the channel is interrupted, making crowd control the first answer when available. Garen's passive waits nine seconds after champion damage; both item durations expire before that out-of-combat regeneration starts, while continued damage prevents it from starting.

If anti-heal seems absent, recheck the documented condition: enemy champion, Basic Attack On-Hit for Executioner's, or magic damage while below 40% Health for Morellonomicon. The live tooltips do not settle the exact threshold-crossing hit, fully shielded damage, every ability that applies On-Hit effects or how simultaneous sources refresh. Those interactions require a controlled live-client test; this guide does not guess at them.

Current on patch 26.19
Executioner's Calling and Morellonomicon are unchanged from the launch client data. None of the three items that returned in 26.19, Cloak and Dagger, Moonflair Spellblade and Zz'Rot Portal, applies Grievous Wounds. Patch 26.16 increased the visibility of healing decisions through changes such as stronger later-rank Mundo regeneration and stronger allied healing from Alistar, while also adding sustain-bearing kits such as Akali and Shen. That context makes anti-heal worth evaluating more often; it does not turn either finished item into an automatic purchase in every one of those games.

Before spending the combine gold, answer six questions: who receives the meaningful healing, when does that recovery happen, can this buyer apply the listed trigger, can they refresh it for the whole window, does the team already have dependable coverage, and which core spike is delayed? If any answer is unclear, watch the next fight before committing. Anti-heal is powerful only when it is active on the right health bar at the right time.
